Four kinds of help, one circle
Each brings something the others can't. Together they help a person reach what they need, further than any of them could alone.
Clinicians and practices
Start with your own practice. A matching tool on your website helps each person who reaches out find the clinician on your team who fits. You can set it up yourself, step by step.
- Your own matching tool, on the website you already have, WordPress included. It never needs a client’s name
- Website widgets you paste in yourself. You don’t need a CRM for any of it
- As the network grows, partners who say who takes which insurance and ages, and who is taking referrals
- Clinical records stay where they already are, in your practice’s own systems
Community organizations
They help people reach what they need: food, housing, rides, a job, government benefits and programs, a group to belong to. Here they keep track of the neighbors they walk with, the partners they send people to, and the follow-ups that cannot slip.
- Contacts: one shared record of the people and partners you work with
- Your partners, searchable in plain words, like “who takes Medicaid”
- Community Challenges: 21 days of a small shared habit, a little each day, together
Educators
They often notice first when a student or a family is having a hard time. Here they can bring the right people in and stay part of the plan, without carrying it alone.
- One conversation with the family and the people helping, instead of scattered emails and calls
- Community partners for what a school can’t provide on its own: meals, rides, tutoring, counseling
- The family in the conversation, so they always know who is helping and what comes next
The people closest to them
They're there on the hard nights and the ordinary days. Here they share one conversation with everyone helping, so nobody has to tell the story twice or wonder who is doing what.
- One shared conversation, on the web or from Messages on an iPhone
- A private notebook beside the chat, for what you are learning and planning
- Somewhere to go, in person: real groups and meetings, each with the date someone last checked it
One person, a few people, one plan
- Maya tells someone.
After a hard month, Maya mentions it to a friend from her running club. The friend doesn’t need to know what to do next, only who to bring in.
- A small circle forms.
The neighborhood nonprofit her friend volunteers with opens one conversation with Maya and her sister. With Maya’s say-so, they look through the partners they already work with and make a warm introduction to a counselor who fits.
- Everyone can see the plan.
A ride on Tuesday, groceries from the food pantry on Thursday, help with a benefits form, the support group she wants to try. It is written down in one place, so her sister isn’t guessing and nobody is calling around.
- Nothing quietly drops.
If someone asks for help and nobody has answered, the team is told, and only the team. Nobody is contacted on Maya’s behalf.
Maya is an example, not a real person.
Automated assistance, with a person behind it
LIAH is the Care Circle’s automated assistance. Ask in plain words: find a partner who takes Medicaid, set up the matching tool on my website, walk me through inviting my team. It is plainly not a person and never pretends to be.
- Walks you through setup, one step at a time
- Finds a partner or a local resource that fits
- Drafts a message, and you decide whether to send it
- Answers only when someone types @liah
Would rather talk to a person? One tap reaches a real person on our team, who gets back to you within 24 hours.
Talk to a personWhat it holds, said plainly
Matching never needs a name
The matching layer never sees a client's name, only what it takes to find a fit: a specialty, an insurance type, an age range, telehealth or in person, and who has an opening. A check runs on every release to keep it that way.
A place to coordinate, not a chart
Clinical records stay in a practice's own systems. The Care Circle is where the people around someone talk and plan.
Notes don't linger
A note on a referral and a message between practices are kept for 90 days, then deleted. Please keep anything private about a person out of free-text boxes.
Free to join, and free to stay
No contract and no per-seat pricing. If we ever add something paid, what you already have stays free.
Read the privacy page or the user agreement for the details.
Built and given freely by Love Is A Habit
A 501(c)(3) nonprofit in Maryland since June 2022. Nobody gets through hard seasons alone, so we build the place where the people around someone can find each other and keep showing up.
Request an invite
The Care Circle grows by invitation, so every group that joins is welcomed by a person. Tell us a little about you and a person on our team will get back to you within 24 hours.
